In this application, we will be asking you several questions that are important for us to understand going into our time together. Please take your time completing this application.
When you book a Journey to Wellness Call, here's the process that you can expect:
#1 You will speak directly with a knowledgable and experienced member of Journey to Wellness.
#2 They will guide you through a full health history.
#3 They will take the time to address your questions.
#4 You will walk away with recommendations on the right testing you need to uncover your root causes.
These labs are not covered by insurance. Depending on which labs our team recommends, cost can range from $500-$1000 for your testing package. Please be prepared to have out of pocket expenses for your testing if you choose to schedule this call.
#5 When your results are in, we'll schedule your test review appointment (included in the cost of your testing package).
#6 During your test review appointment, based on your specific results we'll walk you through which Journey to Wellness program is right for you and go over all associated costs if you choose to move forward.
There is a $49 fee for the call. After the application below is submitted, you'll taken directly to a page where you can pay and schedule your call with us.
